Now that Carmela and Tony are separated, Tony begins to have romantic interest in Dr. Melfi. However, Tony and Carmela cross paths when she's forced to call him after a wild bear arrives at their home. Johnny Sack sees an opportunity to move up when Carmine has a stroke. Meanwhile, Janice adjusts to life married to Bobby, and Christopher and Paulie butt heads when Christopher always has to pay for dinner at restaurants.

Tony throws a party for Tony Blundetto, his cousin recently released from prison on parole. Meanwhile, the FBI continues to press Adriana for information, and she almost breaks down and confesses at Carmela's film club. With Carmine dead, Johnny Sack sees an opportunity for advancement, but Little Carmine may get in the way of that. Elsewhere, Tony seeks out another rat and later leaves a blatant message for the Feds.

Feech crosses the line by closing in on Paulie's landscaping deals. In New York, the hostility escalates, which forces Johnny Sack to send the Leotardo brothers to Little Carmine to deliver a message. Tony begins to worry about Uncle Junior after he finds him searching his old neighborhood for his dead brother.

Tony and Carmela argue about A.J. when he heads to New York for a wild night on the town. Feech continues to cross the line concerning Paulie's landscaping business, forcing Tony to make a decision about him. Meanwhile, a spark arises between Carmela and A.J.'s guidance counselor.

While Christopher is away on business, the Sopranos crew discusses a rumor involving Adriana.

On a sweltering afternoon, the two Tonys are watching a Mets game on the tube. When Tony B. goes to get a beer, Tony S. notices he's favoring his right foot. Two black guys jumped me outside a bar, Tony B. explains. Later, when Tony S. is playing golf with Johnny Sack, he asks whether there's any news on the Joe Peeps shooting. All Johnny can offer is that some homeless **** saw a guy limping away from the murder scene. Soon after that Tony suffers an anxiety attack and collapses on the tee box.

Janice attacks a mother during her stepdaughter's soccer game and is forced to attend anger management classes. Tony must deal with his own anger as Johnny Sack launches payback against the Sopranos. Christopher and Tony B. overcome their differences when they are sent to dispose of several incriminating corpses buried in the country.

After his latest goomar is injured, Tony escapes to the Plaza Hotel for a night of rest and relaxation, but his dreams are haunted by past faces. Another casualty falls in the war between Johnny Sack and Little Carmine, and Tony B. heads to New York for revenge.

The FBI finds new evidence that they use as leverage against Adriana. Adriana relents and tries to persuade Christopher to go into witness protection with her. Tony and Carmela meet over lunch and work toward a reconciliation, which includes a new real estate project for Carmela. Johnny Sack delivers a stern ultimatum to Tony, who is hesitant to give up his cousin.

Tony faces pressure from his own crew to give up his cousin to Johnny Sack. Tony does some soul-searching on Melfi's couch and finally decides on a course of action.
